Franciscan Health partners with Circulation for nonemergency transportation

Mishawaka, Ind.-based Franciscan Health launched a partnership with Circulation, whose digital platform coordinates nonemergency medical transportation for patients with ongoing medical conditions.

Circulation estimates over 3.6 million Americans miss appointments each year due to transportation issues. The Circulation platform is designed to become a single access point for scheduling transportation, with 100 care managers and social workers involved in the discharge process.

If transportation is identified as a barrier for patients at discharge, a care manager will set up a ride to bring the patient home. Then, a care manager can set up that patient's transportation to and from future appointments with primary care providers.

Franciscan Health hospitals are aiming to reduce 30-day readmission numbers after patients' discharges from inpatient stays. The health system determined lack of transportation to be the second leading cause of hospital readmissions.

"This partnership is the most recent demonstration of how Circulation is managing patient logistics from hospital bedside through the cycle of care, extending into the patient's home and downstream to community providers," said Robin Heffernan, CEO and co-founder of Circulation.
