2 Colorado hospitals team up for transitional care

St. Mary's Medical Center in Grand Junction, Colo., and Colorado Canyons Hospital and Medical Center in Fruita launched a program aimed at improving long-term care, according to a Grand Junction Daily Sentinel report.

As part of the Transitional Care Program, St. Mary's patients who are not well enough to go home, but are able to leave the intensive care unit, will move to Colorado Canyons, the report states. In addition, Colorado Canyons is planning a $4.5 million expansion. The expansion, which began construction this week, includes nine additional patient rooms and provides room for growth in the hospital's therapy program and pharmacy, reports Grand Junction Daily Sentinel.

"St. Mary's is happy to supply care so they don't have to go to Salt Lake [City] or Denver. But it's not the best care and it's not because we don't do a great job. It's just not our core competence," St. Mary's President Brian Davidson, MD, said in the report. "This is a solution where patients can get to the right place to get the right care at the right time. We've had the right care, this is the right time and Colorado Canyons is the right place."

Hospital officials expect the Colorado Canyons expansion to be completed by the early part of 2018, according to the report. Once the expansion is completed, Colorado Canyons, a critical access hospital, will have 25 total beds.
