A chronic care clinic backed by a partnership between UNC Health Care and Blue Cross of North Carolina opened last week, according to a system news release.

The clinic — Carolina Advanced Health — is located in Chapel Hill and treats BCBS customers who are over the age of 18 and have a chronic illness, such as hypertension, asthma, chronic heart failure, diabetes and high cholesterol. It is staffed by two primary care physicians and one physician assistant.

"Patients with chronic conditions can spend a great deal of their time visiting doctors in multiple locations and updating them on their symptoms and treatments," Thomas K. Warcup, DO, Carolina Advanced Health medical director said in the release. "Carolina Advanced Health emphasizes a coordinated approach for patients that will improve the quality of care they receive."
