90 Hospital-Affiliated Practices Test Patient-Centered Medical Neighborhood

TransforMED, a subsidiary of the American Academy of Family Physicians, announced it has chosen 90 primary care practices to participate in a three-year patient-centered medical neighborhood pilot project.

The PCMN model connects patient-centered medical home primary care practices with other community-based healthcare providers to improve care coordination and lower costs. The goals of the project include the following, according to the news release:

•    Reduce overall healthcare costs for Medicare and Medicaid beneficiaries by a total of 4 percent, or $49.5 million, by 2015.
•    Improve the health of the eligible population as demonstrated by an average of 15 percent — and at least 3 percent improvement — in each selected quality measure by 2015.
•    Improve patient experience measures that reflect patient engagement, access and quality by 25 percent by 2015.
•    Demonstrate the ability to scale additional practices within each community by 2015.

The 90 primary care practices are affiliated with 15 health systems in 65 cities across the U.S. Here are the 15 health systems:

•    Avera Health (Sioux Falls, S.D.)
•    Charleston (W.Va.) Area Medical Center,
•    Columbus (Ind.) Regional Health
•    Greater Baltimore Medical Center
•    Huntsville (Ala.) Hospital
•    Integris Health (Oklahoma City)
•    Marquette (Mich.) General Hospital
•    North Mississippi Health Services (Tupelo)
•    North Shore Medical Center (Salem, Mass.)
•    Northeast Georgia Health System (Gainesville)
•    Novant Health (Winston-Salem, N.C.)
•    Orlando (Fla.) Health
•    Owensboro (Ky.) Health
•    Via Christi Health (Wichita, Kan.)
•    Western Connecticut Health Network (Danbury)

The project is funded by a three-year, $20.75 million award by the CMS Center for Medicare and Medicaid Innovation. It will be led by TransforMED in collaboration with VHA, population health technology company Phytel and analytics company Cobalt Talon.
