Vermont Information Technology Leaders — which operates the Vermont Health Information Exchange — connected with the Veterans Health Information Exchange.
This connection enables clinicians at VA facilities to view patient records from participating healthcare organizations across Vermont and the surrounding region, and to share health information for the more than 48,000 veterans who live in the state.
The goal of this agreement is to improve transitions of care between veterans and community healthcare facilities.

"Our organizations have been working for more than a year to put this community exchange in place, and we are proud to provide services that will help provide better care for our veterans within the Vermont health system."
