Stellar Health: Finally, a technology tool that actually helps you help patients

Wouldn’t it be great if your electronic health record didn’t just passively absorb information (and time), but instead actively alerted you to incomplete preventative screenings and other care opportunities?

Stellar Health does just that. Stellar is a point-of-care software platform that digests claims, EMR, and hospital data to alert you and your care team to unfulfilled care and coding gaps. On top of that, the Stellar platform allocates additional payments from participating insurers, on top of RVUs, when providers and staff close the identified gaps. Imagine turning your pre-visit huddle with the nurse or MA into a structured, digital or paper-based opportunity to rapidly create value for you and your patients.

One primary care practice in Florida with a complex, elderly patient population implemented Stellar to drive significant benefits for their patients. Diabetes care composite scores (HgbA1C control plus eye exam completion) increased from less than 30% to over 50%. In addition, colorectal cancer screening rates increased from less than 40% to over 65%.

Stellar also bridges the gap between care and making coding clinically useful. One provider said, “Stellar helped us catch a patient with COPD who had been unmanaged for multiple years. As a result of Stellar, we caught the COPD, did spirometry that day, and got the patient started on treatment for COPD -- the patient had been on no treatment for COPD prior to this.”

Before using the Stellar App, physicians would on average address 2.0 high-value chronic conditions per patient; since using the App they complete 3.4 per patient (a 70% increase).

The financial realities of primary care make it impossible not to consider the value Stellar creates for providers as well. For the Florida practice above, the patient panel coding gap shrunk from 0.55 to 0.27, which equated to an additional almost $2,800 of revenue per patient due to improved accuracy for risk-based revenue. As a result, doctors shared in $300 to $1000 per month for completing high-value actions related to prevention.

Technology has enabled many miraculous things in medicine, but our information systems have not kept pace--this despite the fact that they touch more patients than perhaps any tool other than a stethoscope.
