Signify Health to increase social determinants of health initiatives through new acquisition

Signify Health, a healthcare technology company that provides in-home care and management services, acquired technology platform TAVHealth to enhance its social determinants of health initiatives.

While providing in-home care, Signify Health clinicians often identify social determinants of health factors, such as food insecurity and transportation issues, among patients. Providers document this information on Signify Health's technology platform to enable case management referrals.

Signify Health will integrate TAVHealth's technology to its platform to directly manage their clients' social determinants of health needs. Signify will be able to access TAVHealth's network of community-based organizations to enroll clients in programs and services that support their social determinants of health needs.

"A surprisingly high percentage of health outcomes are driven by social, environmental and behavioral factors," Signify Health CEO Kyle Armbrester said in a news release. "While we have rich data on these factors from what our clinicians capture while caring for members in the home, healthcare has historically had a last mile problem on connecting the need with the fulfillment. TAVHealth provides the tools and network to put food in the fridge and keep our most vulnerable members safe and secure in their own homes."

Signify Health comprises a network of more than 4,000 physicians and nurse practitioners who provide in-home healthcare services for more than one million Medicare members annually.
