Last November, a camera containing photographs of women with breast cancer before and after surgery went missing from the hospital’s Plastic Surgery Clinic. Veterans Affairs said the camera, which also included patients’ Social Security information, was never recovered and that the women were offered free credit monitoring, according to ABC.

Other items that were lost or stolen include a $14,000 computer and a thumb drive containing encrypted patient information.
