The first six months of the year saw numerous data breaches. Here are eight of the biggest ones that occurred at hospitals and health systems, as reported to HHS' Office for Civil Rights' breach notification portal.

This roundup includes breaches at healthcare organizations. It does not include third party vendor breaches, such as the breach of Bizmatics' server.

1. 21st Century Oncology (Fort Myers, Fla.): 2.2 million individuals affected (hacking/IT incident)

2. Radiology Regional Center (Fort Myers, Fla.): 483,063 (paper/films loss)

3. Premier Healthcare (Bloomington, Ind.): 205,748 (theft)

4. Community Mercy Health Partners (Springfield, Ohio): 113,528 (improper disposal)

5. Ohio Department of Mental Health and Addiction Services: 59,000 (unauthorized access/disclosure)

6. Eye Institute of Corpus Christi (Texas): 43,961 (theft)

7. Mayfield Brain & Spine Clinic (Cincinnati): 23,341 (hacking/IT incident)

8. Mercy Iowa City (Iowa): 15,625 (hacking/IT incident)

9. Wyoming Medical Center (Casper): 3,184 (hacking/IT incident)
