According to the RFI, next generation models for advanced primary care “would seek to improve further the delivery of patient-centered care and population health and would align with [HHS’] goal to increase the use of alternative payment models.”
CMS seeks comments specifically related to patient continuity, care of patients with complex needs, advanced primary care within ACOs, meaningful performance measurement, multi-payer participation and use of health IT, among others.
CMS is seeking input and comments from consumers, consumer organizations, healthcare providers, associations, purchasers and health plans, Medicaid agencies, health IT vendors and other stakeholders.
Comments are due March 16.
