CloudMedx has added a new analytics platform aimed at improving chronic care management to Medicare beneficiaries.
The new suite of analytics tools help providers and medical practices automatically identify, enroll, track and adjust care for eligible CCM patients through data integration and risk stratification.
Poor CCM utilization is driven by lengthy documentation, lack of data analytics, upfront costs, as well as patient variation and pushback. CloudMedx aims to alleviate these issues by introducing a simplified communication and analytics platform capable of managing chronic care patients within a single application.
Key new features within the platform include patient engagement tools, curated care plan capabilities, seamless workflows and staff communication tools.
