CareCloud, a vendor of cloud-based practice management, medical billing and EHR software and services, has partnered with Lightbeam Health Solutions to integrate population health management tools and data analytics into CareCloud's clinical platform.
The partnership aims to help ACOs, physician groups and independent practices gain insights into patient populations, coordinate care, control utilization and improve clinical outcomes while reducing costs, according to a news release.
It will focus on CareCloud's application platform, and enable medical group clients to analyze data and identify at-risk patients and those in need of proactive intervention across multiple providers and facilities, the release states.
