Patient IO is the latest digital health company to join athenahealth's More Disruption Please accelerator program.
Patient IO is a care coordination solution that supports care management and engagement with patients between visits. Its end goals are to help patients manage chronic illness, reduce readmissions and reduce unnecessary visits to physicians' offices.
