95% of hospitals cite concern over communication during patient discharge: 6 survey insights

Ninety-five percent of hospitals express concerns about communication within the patient discharge planning process, according to a survey by Imprivata and Spyglass.

The survey asked 20 clinical informatics experts about care coordination workflows at their hospitals.

Here are six survey insights.

1. Eighty-five percent of respondents expressed concern about coordinating community-based resources to support patients, family members and care givers after discharge.

2. Forty-five percent of respondents were concerned about accurately identifying and resolving potential conflicts over prescribed patient medications before, during or after hospitalization.

3. Eighty percent of respondents were concerned with internal communication and collaboration amongst members of multi-disciplinary teams.

4. Sixty-five perfect of respondents were concerned about efficiency and safety when transitioning patients from one medical service to another.

5. Fifty percent of respondents were concerned about communicating patient procedures and tests and coordinating transfers to ancillary departments.

6.Seventy percent of respondents are developing or have already developed a clinical communications strategy to address existing inefficiencies.
