6 Northern California hospitals share data to manage frequent fliers in ER

A group of hospitals in California's EastBay have implemented a data-sharing platform to coordinate and improve care for emergency room frequent fliers.

The platform, PreManage ED, is a communication tool that also offers a portable health record and care plan for patients. When a frequent patient registers at any of the six participating hospitals' ERs, all hospitals are alerted that the patient has presented, and care providers from different organizations can coordinate care.

"Patients are already experiencing the benefits of a coordinated approach to care," David English, MD, associate CMIO and an emergency department physician at HighlandHospital in Oakland, Calif., said in a statement. "Continuity of care is critical to long-term wellness and chronic disease management. PreMange ED is a win-win — patients benefit from a more cohesive experience and providers can improve care based on shared knowledge, which contributes to better outcomes."

The six hospitals participating in the data sharing initiative are part of Sacramento, Calif.-based Sutter Health and Alameda Health System in Oakland, Calif. They include Alta Bates Summit Medical Center in Oakland and Berkeley, Sutter Delta Medical Center in Antioch, Eden Medical Center in Castro Valley, Highland Hospital in Oakland and San Leandro Hospital in San Leandro.
