The following data breaches were reported over the past four weeks, beginning with the most recent.

1. Northwestern Memorial Hospital notified 2,800 patients of a data breach after a laptop was stolen from an employee's car.

2. An employee at Richmond-based Virginia Commonwealth University Health System accidentally donated compact discs storing protected health information to a children's art project from approximately January 2012 through October 2014.

3. Clay County Hospital in Flora, Ill., was blackmailed with patient data.

4. An unencrypted laptop was stolen from a Corvallis (Ore.) Clinic employee's car, prompting the clinic to report a breach of patient privacy.

5. Tribeca Medical Center in New York notified patients of a data breach after discovering the theft medical records stored in a storage facility.

6. WellCare Health Plan notified more than 50 patients of New York State of a data breach caused by a vendor's computer coding error.

7. A third-party vendor handling patient information for Highlands-Cashiers Hospital in Highlands, N.C., inadvertently left patient data accessible online, prompting the hospital to notify 25,000 patients that their data may have been at risk.

8. An employee at University Hospitals in Cleveland breached the data of 692 patients by inappropriately accessing their medical records in the hospital system's EHR platform.

9. A former IT contractor for Tacoma, Wash.-based Franciscan Health System stole nearly $100,000 in computer equipment after his employment contract ended, prompting the health system to report a security breach.

10. Northfield (Minn.) Hospital & Clinics notified approximately 1,700 patients that their protected health information may be compromised after documents containing their information were placed in a commercial dumpster.
