Syncordia unveils new product for care coordination: 4 things to know

Syncordia Technologies and Healthcare Solutions, a revenue cycle management company based in Toronto, Canada, has unveiled its proprietary Coordinet product. 

Here are four things to know about the new product.

1. Coordinet is part of Syncordia's "Platform Syncordia" revenue generating forthcoming products.

2. The cloud-based application aims to help hospital systems coordinate care for high-risk patients, according to a news release.

3. This SaaS patient management tool facilitates collaborative documentation of high-risk patients including diagnosis, treatment plans and post-release continuation of care among all of the patient's caregivers. 

4. Coordinet includes:

  • A patient encounter management dashboard
  • A patient follow-up activity tracker with actionable task lists
  • An integrated LACE Tool for identifying high-risk patients. LACE index identifies patients that are at risk for readmission or death within 30 days of discharge.
  • Configurable risk rules
  • Intelligent rule engines to compile and track data
  • Analytics dashboards
  • Integration with hospital EMR systems
