AI-enabled care platform Clarify Health Solutions rolled out a bundled payment and population risk management solution Oct. 31.
Through the platform, providers and payers can access a "care command center." The center provides information on patients' continuum of care that helps providers determine success in a bundled payment or population risk arrangement.
"To confidently take on risk for populations and bundled payments, physicians and health systems need to have patient-level predictive insights to optimize care," said Mark Froimson, MD, president of the American Association of Hip and Knee Surgeons and former chief clinical officer and executive vice president of Livonia, Mich.-based Trinity Health.
