Capacity is one of the clearest margin levers health systems have left, and much of it leaks out in the handoffs. Boarding hours, avoidable days, slow discharges and unavailable beds are often symptoms of fragmented care transitions rather than a true shortage of physical space. In this session, health system leaders will examine where transitions break down across the continuum, what it takes in practice to turn those handoffs into measurable throughput gains, and how remote monitoring and ambulatory data serve as the infrastructure supporting that work. Attendees will leave with a sharper view of how care transitions connect to bed availability, discharge velocity and financial performance.
