Yale New Haven Health to let patients add personal 'stories' into EHRs

Yale New Haven (Conn.) Health has made a new patient engagement tool available to all patients via its MyChart patient portal.

Yale New Haven Health first deployed the solution, developed by PatientWisdom, within select clinical areas and facilities in 2016. Now, the health system has expanded the service to all inpatients and outpatients.

The patient engagement tool aims to help clinicians "understand what matters to patients as people," according to a news release. Using the tool, patients can share "stories" about their interests and preferences for their care, which is then delivered to their provider via the health system's EHR.

"PatientWisdom enables patients to tell their own stories, in their own words, as well as to post a photo and name they would like to be called," explained Michael Bennick, MD, medical director for patient experience at Yale New Haven Health.

"In the process, we learn about their goals and barriers as well as social determinants of health and decision-making styles," he continued. "It gives the clinician a quick snapshot view to help understand the patient as an individual before walking into the room."
