In 2012, Milwaukee-based Advocate Aurora Health Care partnered with Cerner to create the Advocate Cerner Collaborative, which uses data and technology to improve outcomes and reduce costs.
The majority of the collaborative's work, heavily reliant on the field of data science, has involved using EHR and claims data to conduct statistical analyses and build predictive models, with a goal of better matching patients to the most effective interventions. The department has also developed a now-widely used readmission risk score that is updated continuously throughout each patient's hospital stay based on EHR data, resulting in significant reductions in readmissions.
Other initiatives include the development of an algorithm that recommends the most appropriate post-acute care setting for patients in need of transitions of care; predictive models to identify patients best suited to an outpatient care management program; and visualizations to help clinicians understand complicated fill histories, targeting patients with low or mixed medication adherence.
The Advocate Cerner Collaborative is staffed by 11 specialists. Their work has resulted in significant improvements in care quality and outcomes, including a more than 20 percent reduction in acute encounters for patients selected by the collaborative's predictive model to join a heart failure outpatient care management program.
