Arizona Medicaid alerts 3,100 enrollees of privacy breach

Officials within Arizona's Medicaid program reportedly sent personal health information of 3,146 patients to incorrect home addresses, according to AZ Central.

An investigation began after an error caused IRS 1095-B forms to be directed to the wrong addresses, state officials told the online publication.

Arizona’s Medicaid program, the Arizona Health Care Cost Containment System, has notified the affected patients.

The forms contained names and dates of birth. No Social Security numbers were compromised.

Arizona Medicaid provides benefits to 1.87 million people.
