10 hospitals with highest, lowest heart failure readmission rates

Lancaster (Pa.) General Hospital has the lowest 30-day readmission rate for heart failure in the country, according to CMS data.

CMS collected provider data for the hospital return days, unplanned readmissions and measures of unplanned hospital visits after outpatient procedures. Data was collected from July 2019 through June 2022.

Here are the hospitals with the highest and lowest 30-day readmission rates for heart failure (Note: Data from the first and second quarters of 2020 were not reported due to the pandemic):

2. Asante Rogue Regional Medical Center (Medford, Ore.): 15.9

3. Carson Tahoe Regional Medical Center (Carson City, Nev.): 16.3

4. Mckay Dee Hospital (Ogden, Utah): 16.4

5. Straub Clinic and Hospital (Honolulu): 16.6

5. Borgess Medical Center (Kalamazoo, Mich.): 16.6

5. Memorial Healthcare System (Chattanooga, Tenn.): 16.6

5. Centra Health-Lynchburg (Va.) General Hospital: 16.6

5. Sentara Martha Jefferson Hospital (Charlottesville, Va.): 16.6

10. Verde Valley Medical Center (Cottonwood, Ariz.): 16.7

10. St. Luke's Regional Medical Center (Boise, Idaho): 16.7
