Primary Partners ACO, Cigna launch collaborative care initiative

The Bloomfield, Conn.-based health insurance giant Cigna started a fifth collaborative care initiative in Florida with the Clermont-based Primary Partners accountable care organization, according to a company release.

The initiative, called Cigna Collaborative Care, began Oct. 1 to help Primary Partners achieve its population health goals through improved care coordination and accessibility. Primarily through the use of care managers, the program aims to reduce missed health screenings or prescription refills, improve preventive care and implement more patient follow-ups. The clinical care managers are registered nurses employed by Primary Partners. They will use patient data from Cigna to identify individuals who may need health screenings, follow-ups, lifestyle or disease management and health education, according to the release.  

More than 8,000 Cigna beneficiaries who are also patients of Primary Partners physicians will automatically receive the benefits of the program. Other Cigna beneficiaries can access the program by going to a Primary Partners physician, according to the release.

Primary Partners is led and managed by 60 healthcare providers in Central Florida. The organization was one of the first Medicare Shared Savings ACOs.
