Phoenix Children's Care Network, UnitedHealthcare launch ACO

Phoenix Children's Care Network rolled out an ACO with UnitedHealthcare aimed at improving quality while reducing costs.

Under the program, UnitedHealthcare will share data with PCCN regarding patients' underlying medical conditions, past medical care, potential gaps in care, prescriptions and future treatment. The organizations intend for data sharing to curb duplicative tests and streamline care coordination across providers.

"This collaboration also improves the ability to identify patients at high risk, and help them reduce emergency room visits and readmissions to the hospital, manage their chronic health conditions and take their needed medications," PCCN and UnitedHealthcare said.

Roughly 50,000 UnitedHealthcare members enrolled in a Medicaid plan are eligible for the program.

PCCN's clinically integrated network comprises more than 100 physicians.
