Mount Carmel Health Partners, a physician hospital organization jointly owned by Columbus, Ohio-based Mount Carmel Health System and 1,500 physicians, and UnitedHealthcare have formed an accountable care organization, effective Oct. 1.
The ACO will cover more than 20,000 participants in UnitedHealthcare employer-sponsored health plans who currently receive care from Mount Carmel Health Partners providers.
Under the ACO agreement, participating primary care physicians will receive monthly updates on their patients to help monitor any care the patient receives. Care providers in the ACO can receive payment incentives based on improvements in things such as readmission rates, patient safety and disease management and prevention.

This is Mount Carmel's second ACO program. It is the first ACO UnitedHealthcare has formed in Ohio.
