Humana, North Shore-LIJ to coordinate care for 1k Medicare Advantage members

Humana and Great Neck, N.Y.-based North Shore-LIJ Health System announced Wednesday an agreement to launch a value-based collaboration to improve quality and reduce costs by coordinating care for more than 1,000 Medicare Advantage members in New York City and Long Island, N.Y.

The value-based care plan is set to begin Jan. 1, 2016. Care will be coordinated through the 5,500-physician integrated network North Shore-LIJ Premium, and Humana's Medicare Advantage members will continue to have access to the North Shore-LIJ network, which spans 19 hospitals and more than 400 outpatient practices.  

"We are proud to collaborate with Humana in the New York market, further increasing our value-based contracting," Howard Gold, executive vice president and chief managed care business officer at North Shore-LIJ, said in a statement. "North Shore-LIJ and Humana are both committed to serving the Medicare population with exceptional care and service. The value-based plan allows us to focus on measurable patient outcomes, care coordination and aligning quality with payment incentives."
