65 accountable care, shared savings agreements in 2016

The following list includes ACOs and collaborative care agreements that were announced, launched, ended or expanded in 2016, beginning with the most recent.

Note: This is not an exhaustive list. It includes the ACO and collaborative care agreements covered by Becker's throughout 2016.

1. Christiana Care Quality Partners ACO adds 2 health system partners, rebrands
Wilmington, Del.-based Christiana Care Quality Partners ACO is expanding and changing its name to eBrightHealth ACO on Jan. 1, 2017.  

2. Highmark BCBS, Aledade strike two-state ACO partnership\
Bethesda, Md.-based Aledade is expanding two Medicare Shared Savings Program ACOs in Delaware and West Virginia to cover Highmark Blue Cross Blue Shield members.  

3. Community Healthcare System to form collaborative care relationship with Cigna
Munster, Ind.-based Community Healthcare System and Bloomfield, Conn.-based Cigna entered into a collaborative care relationship to improve access to healthcare, enhance care coordination and lower costs for residents of northwest Indiana covered by Cigna.

4. BCBS of Kansas partners with Aledade ACO
Bethesda, Md.-based Aledade's Kansas ACO — Aledade Kansas — and Blue Cross and Blue Shield of Kansas partnered to offer value-based contracts to ACO members beginning Jan. 1, 2017.  

5. UnitedHealth Group to roll out national ACO in 2017
UnitedHealth Group plans to launch a national ACO called NexusACO beginning in 2017. 

6. Vermont gets OK for all-payer model
Vermont's regulatory healthcare board approved a statewide all-payer ACO model to improve healthcare quality, coordination and spending.

7. UNC Health, Cigna launch collaborative care initiative
Cigna is teaming up with Chapel Hill, N.C.-based UNC Health Care and its clinically integrated network, UNC Health Alliance, to launch a collaborative care program similar to an ACO beginning Jan. 1, 2017. 

8. Doylestown Health, Holy Redeemer plan to roll out Medicare ACO in 2017
Doylestown (Pa.) Health and Meadowbrook, Pa.-based Holy Redeemer Health System applied to participate in the MSSP beginning in 2017.

9. Delaware Valley ACO, Humana renew value-based contract
Delaware Valley ACO and Humana renewed their value-based network agreement for Humana's Medicare Advantage members. 

10. New York launches first Medicaid ACO
Buffalo, N.Y., is home to the state's first ACO designed to serve Medicaid and privately insured patients, reports Buffalo News. 

11. Blue Shield of California, PrimeCare, San Antonio Regional expand Calif. ACO
Blue Shield of California is collaborating with PrimeCare Medical Network and Upland, Calif.-based San Antonio Regional Hospital to expand its ACO in Southern California.

12. Aledade to roll out physician-led ACO in suburban Philadelphia
Bethesda, Md.-based Aledade added yet another ACO to its portfolio, this time in suburban Philadelphia.

13. Aledade accountable care frenzy continues
On the heels of four other recent partnerships, Bethesda, Md.-based Aledade rolled out a new ACO in Arkansas and expanded its ACO in Missouri. 

14. New Aetna network combines Duke, WakeMed providers
Aetna established an ACO with Durham, N.C.-based Duke Health and Raleigh, N.C.-based WakeMed Health & Hospitals. The resulting product will be called Aetna Whole Health-Duke Health & WakeMed.

15. Mount Sinai CIN, Cigna form accountable care partnership
New York-based Mount Sinai Health Partners and Cigna are teaming up to offer Cigna beneficiaries better access to more coordinated care at a lower cost through a collaborative care agreement. 

16. UCSF, John Muir add 7 hospitals, 3 medical groups to ACO
Walnut Creek, Calif.-based John Muir Health and UCSF Medical Center in San Francisco expanded their ACO.

17. BCBS of Kansas, St. Francis launch ACO
Kansas' largest payer is teaming up with Denver-based SCL Health to roll out an ACO for SCL subsidiary, Topeka, Kan.-based St. Francis Health. 

18. Essentia, Medica launch ACO
Duluth, Minn.-based integrated health system, Essentia Health, and Minnesota payer Medica are teaming up to launch an ACO in Minnesota, North Dakota and Wisconsin beginning Nov. 1.

19. 3 Next Generation ACOs head for the door
Out of the original 21 participants in CMS' Next Generation ACO Model, 18 remained after roughly seven months in the program. 

20. Aetna, Gateway Health in Virginia strike ACO agreement
Hartford, Conn.-based Aetna and Danville, Va.-based Gateway Health formed a commercial, product-based ACO in Virginia. 

21. Cigna, Calif. physicians group ink coordinated care contract
Cigna and Hill Physicians Medical Group in San Ramon, Calif., entered into a collaborative care agreement to improve patient health by improving patient access to quality healthcare services. 

22. UConn, Hartford Health to team up on accountable care
Farmington-based UConn Health and Hartford (Conn.) HealthCare are working together to form an ACO. 

23. Aledade signs first commercial ACO contract with BCBS
With two years of operation under its belt, Aledade, a startup dedicated to helping physicians launch Medicare ACOs, announced a new kind of partnership. 

24. DeKalb PHO, UnitedHealthcare launch ACO
Atlanta-based DeKalb Physician Hospital Organization and UnitedHealthcare are allotting more resources to coordinate patient care in Georgia.  

25. UnitedHealthcare, Aurora team up for a Medicare Advantage ACO
Milwaukee-based Aurora Health Care and UnitedHealthcare of Wisconsin are rolling out an ACO for more than 40,000 Milwaukee residents with Medicare Advantage.

26. Aledade rolls out 3 new ACOs, expands another
To ring in its second anniversary, Aledade added three new ACOs and expanded another.

27. 10 W.Va. health centers team up to form ACO
Aledade launched an ACO led by 10 West Virginia-based Federally Qualified Health Centers. 

28. MemorialCare extends ACO to Boeing employees
Fountain Valley, Calif.-based MemorialCare Health System is teaming up with the Chicago-based Boeing Company to offer a customized health plan for the aerospace company's employees and dependents in Long Beach, South Bay and Orange County, Calif. 

29. Mount Sinai Health Partners, Humana partner on value-based care model
New York City-based Mount Sinai Health Partners and Humana partnered to create a value-based care model that aims to improve health outcomes and care coordination while reducing healthcare costs for Humana Medicare Advantage members. 

30. Memorial Hermann Health System, UnitedHealthcare launch ACO
Houston-based Memorial Hermann Health System and UnitedHealthcare formed an ACO aimed at improving the coordination of healthcare and patient satisfaction.

31. Oneida to participate in Basset Healthcare's MSSP, commercial ACOs through population health partnership
Oneida (N.Y.) Healthcare is teaming up with Cooperstown, N.Y.-based Basset Healthcare Network to enhance primary care and population health management in the Oneida region. 

32. John Muir Health System, Humana sign value-based care agreement
Walnut Creek, Calif.-based John Muir Health System and Humana inked a value-based care agreement that will benefit Humana Medicare Advantage members in Contra Costa County, Calif.

33. Cigna, Integrated Independent Physicians Network roll out collaborative care agreement
Cigna and Orlando, Fla.-based Integrated Independent Physicians Network partnered April 1 to improve quality and access to care and lower costs for 7,000 Cigna beneficiaries. 

34. Evolent Health, Georgia Physicians for Accountable Care strike partnership
Arlington, Va.-based Evolent Health partnered with Macon-based Georgia Physicians for Accountable Care, the largest physician-led ACO in the state. 

35. Cigna, New England Quality Care Alliance form accountable care partnership
Braintree, Mass.-based New England Quality Care Alliance is teaming up with Cigna on a new collaborative care initiative for more than 10,000 patients. 

36. Cigna, Rush launch collaborative care program
Cigna and Chicago-based Rush Health established a new collaborative care relationship similar to an ACO.  

37. Medica, Altru Health team up on ACO in Minnesota, North Dakota
Minneapolis-based Medica and Grand Forks, N.D.-based Altru Health System plan to roll out an ACO called Altru & You with Medica.

38. Integra, Unitedhealthcare launch Rhode Island's first Medicaid ACO
Integra Community Care Network, a Providence, R.I.-based, CMS-certified ACO, is teaming up with UnitedHealthcare Community Plan of Rhode Island to roll out a Medicaid accountable care entity through the State of Rhode Island Accountable Entities Pilot.

39. Aetna teams up with St. John Health System for accountable care
Aetna and Tulsa, Okla.-based St. John Health System, part of St. Louis-based Ascension, unveiled an ACO in Tulsa County.

40. University Hospital, BCBS of NJ to collaborate on value-based care
Horizon Blue Cross Blue Shield of New Jersey and University Hospital in Newark, N.J., are entering into a strategic partnership designed to accelerate the integration of Horizon's value-based care models into University Hospital's operations.

41. Northwell, EmblemHealth sign collaborative care agreement
Building on their two decade relationship, Great Neck, N.Y.-based Northwell Health and New York City-based EmblemHealth partnered in a risk-sharing agreement.

42. Wake Forest Baptist, UnitedHealthcare team up on accountable care
Winston-Salem, N.C.-based Wake Forest Baptist Medical Center, with the help of its clinically integrated network and UnitedHealthcare, launched an ACO.

43. Meritain, Banner to roll out accountable care in Phoenix
Phoenix-based Banner Health Network and Meritain Health, an Aetna subsidiary, are collaborating to offer patients in the Phoenix area highly coordinated care through an ACO.

44. UnitedHealthcare, Delaware Valley ACO partner up
Philadelphia-based Delaware Valley ACO signed another accountable care agreement with a commercial payer, following its deal with Aetna in February. 

45. Aetna, Virtua Health team up for ACO, health plan
Aetna and Marlton, N.J.-based Virtua Health will collaborate to form an ACO and develop co-branded commercial health plans for businesses and consumers.  

46. DeKalb Physician Hospital Organization, Humana launch value-based care agreement
Atlanta-based DeKalb Physician Hospital Organization and Humana expanded their relationship by adding value-based care provisions to their agreement serving Humana commercial members with the goal of enhancing the coordination of care services, improving quality and reducing healthcare costs.

47. BCBS of Rhode Island, Lifespan and Coastal Medicine forge accountable care agreement: 3 things to know
Blue Cross and Blue Shield of Rhode Island, Lifespan and Coastal Medical, all based in Providence, entered into an accountable care agreement designed to improve the coordination of primary, specialty and inpatient care. 

48. 13 rural hospitals to form ACO

Hamilton, Mont.-based Marcus Daly Memorial Hospital and the local medical group, Ravalli Family Medicine, are banding together with a dozen other rural hospitals in Montana, Idaho and Wyoming, to launch an ACO.

49. UnitedHealthcare, Florida Accountable Care Services launch ACO
UnitedHealthcare and Orlando-based Florida Accountable Care Services are launching a joint accountable care program April 1 to improve patients' health and satisfaction while lowering costs. 

50. Dickinson County Healthcare joins ACO
Iron Mountain, Mich.-based Dickinson County Healthcare System joined an ACO.

51. Aetna, Mount Sinai Health Partners launch ACO alliance
Aetna and New York City-based Mount Sinai Health Partners, a clinically integrated network including Mount Sinai Health System and select physicians, inked a three-year deal to enhance coordinated care and lower costs through an accountable care agreement. 

52. River Health ACO drops out of Next Generation program
Just weeks after its selection for the Next Generation ACO program, Harrisburg, Pa.-based River Health ACO chose to leave the program because it would not be able to meet the required cost targets.

53. Health Net, University of California expand ACO to employees
Los Angeles-based Health Net of California brought accountable care to employees at University of California campuses in Davis, Irvine and San Diego in 2016.

54. Cigna, Sequent Health Physician Partners roll out accountable care agreement
Lawrenceville, Ga.-based Sequent Health Physician Partners teamed up with payer Cigna for its first accountable care program, which launched in January 2016. 

55. Cigna adds 5 collaborative care agreements in Florida
Cigna expanded its collaborative care programs to 75,000 more Floridians through five new agreements that launched Jan. 1, 2016.

56. Aetna, Meridian Health form product-based ACO: 4 things to know
Aetna and Neptune, N.J.-based Meridian Health created a co-branded, commercial, product-based ACO.

57. Florida Blue, Palm Beach ACO partner up
Florida Blue and Palm Beach ACO, based in West Palm Beach, Fla., are teaming up to provide high quality, efficient care in South Florida.  

58. Aetna teams up with Delaware Valley ACO
Aetna inked an accountable care agreement with Villanova, Pa.-based Delaware Valley ACO for roughly 70,000 commercial customers in Southeastern Pennsylvania. 

59. 2 NC hospitals join National Rural ACO consortium
Two hospitals in North Carolina's Surry County joined the National Rural Accountable Care Consortium. 

60. Heritage California ACO to exit Pioneer program
Heritage California ACO, an affiliate of Marina del Rey, Calif.-based Heritage Provider Network, will transition from the Pioneer accountable care program to the Next Generation ACO model.  

61. Humana, ConcertoHealth launch value-based care in Detroit
Detroit-based ConcertoHealth teamed up with Humana to launch a value-based care program for Medicare Advantage patients in Detroit and Southeast Michigan. 

62. Cigna, WakeMed Key Community Care Partners launch collaborative care initiative
Cigna is partnering with an already-existing ACO, Raleigh, N.C.-based WakeMed Key Community Care, to enhance healthcare access, care coordination and affordability for Cigna beneficiaries in the Triangle area of North Carolina. 

63. Cigna, Health Choice Preferred roll out ACO
Phoenix-based Health Choice Preferred, an ACO and division of Franklin, Tenn.-based IASIS Healthcare, is teaming up with Cigna to roll out more coordinated care for more than 16,000 patients with commercial health plans in Utah.

64. 100 new ACOs join MSSP
CMS' MSSP added 100 new ACOs and renewed contracts with nearly 150 ACOs on Jan. 1. 

65. CMS announces Next Generation ACO participants
CMS officially launched its Next Generation ACO Model with the selection of the first 21 participating ACOs.
