Valley Presbyterian Hospital receives $100k for ED medication reconciliation program

Van Nuys, Calif.-based Valley Presbyterian Hospital received a $100,000 grant from the California Community Foundation for the implementation of a medication reconciliation pilot program in the emergency department.

The program aims at increasing patient safety. It will reduce the risk of medication order duplications, omissions, dosage errors and drug interactions by ensuring that each patient's medication orders are compared to the medications the patient has been taking.
