Southeastern Pennsylvania Hospitals Join Other Providers to Control Readmissions
Southeastern Pennsylvania hospitals, physicians and other providers are joining together in a new initiative to reduce hospital readmissions, according to a report by the Philadelphia Business Journal.
The goal of the Preventing Avoidable Episodes Project is to reduce readmission rates by 10 percent over 18 months. Three workgroups will create strategies for reducing readmissions.
The project is funded by Independence Blue Cross and southeastern Pennsylvania hospitals, with the Delaware Valley Health Care Council providing data collection and reporting. It is part of the Partnership for Patient Care, a multi-year patient safety and quality initiative facilitated by the Health Care Improvement Foundation.
