Trinity Health, Heritage Provider Network form population health joint venture

Livonia, Mich.-based Trinity Health and Marina del Rey, Calif.-based Heritage Provider Network announced today an agreement to form a population health joint venture, called Trinity Health Partners.

Through the joint venture, the organizations plan to build care networks in specific markets using new models of care to improve coordination and integration. The venture will use Heritage's coordinated care expertise to complement Trinity's existing population health initiatives, which include clinical integration partnerships, value networks and accountable care organizations.

Heritage Provider Network is the largest Pioneer ACO and works with more than 4,000 primary care physicians and 33,000 specialist physicians in California, New York and Arizona. Trinity Health operates 86 hospitals and 128 continuing care facilities in 21 states and is one of the largest nonprofit health systems in the country.

"Populations that are more complex, vulnerable and more in need of health management will be our initial focus," said Paul Harkaway, MD, Trinity's senior vice president of clinical integration and accountable care. Dr. Harkaway expects the model to expand to cover more patient populations over time.

Trinity Health Partners will implement proven Heritage strategies like developing physician leaders in regions around Trinity hospitals and using technology for best in practice outcomes, said Richard Merkin, MD, founder and CEO of Heritage Provider Network.

"We expect it will optimize patient management and provide better person centered care," Dr. Merkin said.
