The Illinois Rural Community Care Organization has partnered with RoundingWell — a healthcare software provider — to deliver patient engagement and care management software to 24 critical access and rural hospitals, 35 rural clinics and 14 rural physician practices across the state.
The RoundingWell care management software program is designed to facilitate population health initiatives, engage patients in the care process and measure quality.
"IRCCO has made great strides in the last year toward understanding and improving the health of our beneficiaries," said Pat Schou, executive director of the IRCCO. "RoundingWell is the final piece of the puzzle that will enable us to engage and coordinate care for our higher-risk patients by using standardized protocols that are accessible to all care coordinators at our 24-plus locations."
