The following is a list of hospitals with the highest proportion of patients who reported they were given information about how to care for themselves after hospital discharge. Data is taken from CMS' Hospital Compare. Hospitals with insufficient patient survey data were excluded from the data sorting process. For reference, the national average rate of discharge information provision is 85 percent, according to Hospital Compare.
