UMass Medical School Awarded Contract to Help Manage Rhode Island PCMH

The University of Massachusetts Medical School has been awarded a contract to support a patient-centered medical home known as the Rhode Island Chronic Care Sustainability Project.

UMass Medical School will provide support in three areas: program implementation, project direction and project administration, which includes budgeting, contracting and communications for the project.

Five years in the making, the Rhode Island Chronic Care Sustainability Project is a coalition of payors, providers, purchasers, consumers and other healthcare leaders with an aim to establish patient-centered medical homes in the state and improve primary care for patients with chronic illnesses.

"Establishing effective patient-centered medical homes is a critical step in closing gaps in care and ensuring that physician practices have the resources and infrastructure necessary to focus fully on patient well-being," said Thomas Bledsoe, MD, co-chair of the initiative. "UMass Medical School's experience in leading patient-centered medical home projects will add tremendous value to the work we have already done."
