Seymour, Ind.-based Schneck Medical Center is assuming operations of Jackson Park Family Physicians and its partner, Urgent Care of Seymour, according to The Seymour Tribune.
As part of the agreement, the practices will take on the hospital's name this weekend. Jackson Park Family Physicians will be renamed Schneck Primary Care, and Urgent Care of Seymour will be renamed Schneck Urgent Care, according to the report.
After a long relationship with the hospital, the physician practice agreed to further integrate with the health system to help make care transitions more seamless, according to the report.
