HHS grants $35.7M in PPACA funding to support patient-centered medical homes in health centers

HHS announced $35.7 million in Patient Protection and Affordable Care Act funding to 147 health centers to support the construction and renovation of patient-centered medical homes in health centers.

"Today's awards will support 21 new construction projects and 126 alteration and renovation projects at health centers across the United States as they work to improve their design and expand existing structures to better support team-based care," said Mary Wakefield, PhD, RN, administrator of HHS' Health Resources and Services Administration.

The patient-centered medical home delivery model aims at improving quality of care through team-based coordination of care, treating the many needs of the patient at once and increasing access to care.
