CMS began reimbursing providers who actively manage care delivery for Medicare patients with two or more chronic conditions through the Chronic Care Management Program Jan. 1. According to a survey from Kryptiq, a population health management company, 92 percent of providers intend to deliver reimbursable CCM services to eligible Medicare beneficiaries within the next year.
Kryptiq conducted the survey among nearly 200 providers from independent practices, integrated delivery systems and clinically integrated network organizations in January.
More than three-fourths (76 percent) of respondents reported they plan to meet chronic care management program requirements within the next six months. These requirements include use of a certified EHR, attainment and management of patient consent, delivery of five core care management services and at least 20 minutes of follow-up care outside of the office.
While the vast majority of providers intend to implement Medicare's CCM program, 43 percent indicated they do not currently have the staff or resources required to effectively carry out the program. An additional 26 percent reported they are currently focusing on the completion of different health IT initiatives, while 15 percent said they are concerned they do not have the right mix of technology in place, according to the report.
