Hospital, pharmacy C-suite leaders on future of healthcare and ACA under Trump: 7 survey findings

More than half of hospital C-suite leaders want to see value-based reimbursement kept intact if President Donald Trump's administration repeals or changes provisions under the ACA, according to a new survey.

For the survey, healthcare performance improvement company Vizient asked hospital and pharmacy C-suite leaders for their opinions and concerns about the future of healthcare and the ACA under President Trump. The survey was distributed to individuals with C-suite or pharmacy executive titles across Vizient's hospital membership in December 2016. A total of 222 leaders responded to the survey.

Here are seven survey findings.

Hospital executives and the ACA

1. Most (89.5 percent) of hospital C-suite leaders want to keep the provision of the ACA that guarantees protection of patients with pre-existing conditions, the survey found. A majority (68 percent) want to see incentives for expanded Medicaid coverage kept intact, 56.4 percent want to keep subsidies to help people purchase insurance coverage and 51.7 percent want to keep value-based reimbursement.

2. However, less than half (45.3 percent) of hospital C-suite leaders want to keep the ACA provision mandating insurance coverage for all Americans, and 40.1 percent want to see alternative payment models, such as bundled payments, kept intact.

3. Assuming President Trump's administration keeps the ACA in whole or in part, 38.4 percent of hospital C-suite leaders believe alternative payment models, such as bundled payments, should be eliminated, according to the survey. A similar number (37.8 percent) believe the ACA provision mandating insurance coverage for all Americans should be eliminated.

4. Nearly all (96.2 percent) of pharmacy executives want to keep the provision of the ACA that guarantees protection of patients with pre-existing conditions, the survey found. However, less than half (46.2 percent) want to keep subsidies to help people purchase insurance, and 38.5 percent want to see value-based reimbursement kept intact.

5. Assuming President Trump's administration keeps the ACA in whole or in part, 61.5 percent of pharmacy executives believe the ACA provision mandating insurance coverage for all Americans should be eliminated. More than 30 percent believe alternative payment models, such as bundled payments, should be eliminated.

Future of healthcare under Trump

6. When asked what gives their facility the most cause for concern following the presidential election, 36 percent of survey respondents said lower reimbursements, while 15.3 percent said fewer insured/covered patients and 14.4 percent said "fear of the unknown."

7. As far as addressing drug price spikes and ongoing drug shortages, more than half (55 percent) of survey respondents want to see President Trump's administration work with the Food and Drug Administration to fast-track competitive drugs to market, while 41 percent want to eliminate the "pay for delay" practice of branded pharmaceutical companies. More than 35 percent want to allow importation of foreign pharmaceuticals.
