Connecticut Joint Replacement Institute Struggles to Implement Bundled Payments

The Connecticut Joint Replacement Institute has struggled to get its bundled payment model accepted by commercial payors, according to the Hartford Business Journal.

Two years after developing a model call the Step Ahead Plan to bundle payments for total hip and knee replacements, Steven Schutzer, MD, co-director of CJRI finally found an insurance company to buy into the model.

The Step Ahead Plan, which is a collaborative effort with St. Francis Hospital and Woodland Anesthesiology Associates, was recently accepted by Farmington-based ConnectiCare to reimburse one value-based price.

For now, any cost overruns get reimbursed based on a fee schedule. Dr. Schutzer called the plan "baby steps" establishing a continuum of care. So far, 20 surgeries have been performed since the contract went to ConnectiCare, and there have been no cost overruns.
