Solera Health rolls out mobile app for diabetes prevention

Solera Health, a Phoenix-based preventive health network that connects patients with payers and physicians to manage chronic diseases, has launched a mobile app, called SoleraONE.

SoleraONE, which is powered by the mobile health company HealthSlate, was developed as a complement to in-person community diabetes prevention programs. These in-person programs connect diabetes patients with lifestyle coaches and group activities, which might include education about healthy eating and physical activity.

The digital component aims to sustain participant engagement, by allowing users to complete make-up sessions when they cannot attend the diabetes prevention program session in person. The app also provides a way to electronically connect with their lifestyle coach between in-person sessions and app-based nutrition, weight and activity monitoring.

"In Solera's experience, consumers choose equally among community and digital DPP providers with similar outcomes," said Brenda Schmidt, CEO of Solera Health. "However, community DPP providers have not had all of the tools and resources required to maximize efficiency to support frequent coach and group interaction between in-person sessions."
