Tampa, Fla.-based DAS Health Partners has partnered with Tampa, Fla.-based LinkedMD to add services in chronic care management.
DAS Health will now utilize LinkedMD's CCM program to give its clients — including physician practices, hospitals, medical groups and ACOs — a set of solutions that will benefit patients.
DAS Health is a leading provider of health IT and management solutions. LinkedMD provides chronic care management services through CPT Code 99490 as encouraged by Medicare.
