AMA spinout Health2047 launches health plan to address social determinants of health

American Medical Association innovation arm Health2047 unveiled its new community-focused and analytics-driven Medicare Advantage plan, which aims to addresses social determinants of health.

Zing Health's foundation is built on the patient-physician relationship, which it uses to connect customers with health services. With this approach, Zing Health highlights the importance of understanding how social context impacts an individual's unique barriers to health.

Zing Health's Medicare Advantage plan can be personalized by each patient to help find community ambassadors, care providers and hospitals. Additionally, each Zing Health customer will have access to a local field care team that collaborates with partner hospitals to coordinate timely procedures and appropriate discharge practices.

By partnering with local providers and physicians, Zing Health will collect data from health assessments and socioeconomic situations that impact health to lower healthcare risks for its members and provide physicians with resources to improve population health management.

AMA launched Health2047 in 2016. The enterprise partners with technology and healthcare leaders to create digital solutions to improve the following areas within the healthcare industry: data liquidity, chronic care prevention and value-based payment.
