8 findings on effective IT and population health management

The vast majority of healthcare professionals (95 percent) believe successful care coordination leads to reduced readmissions, but four in 10 do not think EHRs are sufficient for coordination, according to a survey by PerfectServe.

Harris Poll conducted the online survey for PerfectServe, a healthcare communications platform, gathering responses from 955 medical professionals.

Here are six more key findings on the intersection of IT and population health management and care coordination.

  • Nine out of 10 respondents said investment in sophisticated data analytics is necessary for effective communication.
  • Ninety-five percent of respondents believe interoperability communication technology is necessary in the care continuum, and 88 percent believe mHealth communication technology is integral to that environment.
  • Of the 40 percent of respondents who do not believe EHRs are sufficient for care coordination and collaboration, 53 percent of nurses and care managers indicate EHRs are not adequate for effective communication with physicians.
  • Approximately 75 percent of respondents indicated their organization has invested in patient engagement communication technology, and 74 percent said their organization currently uses online patient portals to optimize population health management.
