The following are the 10 largest healthcare data breaches from 2014 due to hacking or an IT incident based on the number of people affected.

The data come from the HHS' "Wall of Shame" database, where providers are required to report data breaches of unsecured protected health information affecting 500 or more individuals.

1. NRAD Medical Associates (New Hyde Park, N.Y.): 97,000 affected

2. Centura Health (Englewood, Colo.): 12,286

3. Health Texas Provider Network (Dallas): 2,742 affected

4. Baylor Medical Center at Irving (Texas): 2,308 affected

5. Baylor Regional Medical Center at Plano (Texas): 1,981 affected

6. Hospitalists of Brandon (Tampa): 1,831 affected

7. Dekalb Health (Auburn, Ind.): 1,361 affected

8. Midwest Orthopaedics at Rush (Chicago): 1,256 affected

9. Baylor Medical Center at McKinney (Texas); 1,253 affected

10. Blue Cross Blue Shield of Michigan/Blue Care Network: 502 affected
