An early evaluation of the Community-based Care Transitions Program, a Patient Protection and Affordable Care Act test initiated by CMS in 2011 aimed at reducing the number of hospital readmissions by high-risk Medicare beneficiaries, shows the program has not always had success.
Under the program, which is run by HHS' innovation lab, organizations such as community agencies on aging receive federal reimbursement to reduce readmissions.
But only four out of 48 CCTP sites studied saw a significant reduction in readmission rates compared to the 2010 baseline rate, according to the early evaluation, which was produced by consulting firm Econometrica.
However, the early findings were based on evaluation activities underway during the initial project year, so it is not an indicator of overall program success, according to the report.
