Pacifica Hospital of the Valley Joins ApolloMed ACO

Pacifica Hospital of the Valley, a 231-bed hospital in Sun Valley, Calif., has signed a hospital accountable care organization participation agreement with ApolloMed ACO, the Medicare ACO of Glendale, Calif.-based Apollo Medical Holdings.

 

Under the agreement, ApolloMed will share clinical data to help Pacifica Hospital better manage high-risk patients and coordinate care across the continuum.

"We have made the decision to commit ourselves to reaching beyond the four walls of our hospital," Ayman Mousa, DHA, CEO of Pacifica Hospital, said in the news release. "The innovative collaboration between multiple providers, payers and hospitals reflects the uniqueness of ApolloMed's approach to accountable care. We are extremely pleased to partner with ApolloMed to implement this important initiative."

Pacifica Hospital is the fifth organization to join ApolloMed ACO in the last few months: The ACO has added Westminster, Calif.-based Bolsa Medial Group; Santa Ana, Calif.-based Integrated Healthcare Holdings; Garden Grove, Calif.-based Physician Partners IPA and AMVI Medical Group in Orange County, Calif., to its network recently.
