Montefiore Medical Center in the Bronx, N.Y., has formed an accountable care agreement with UnitedHealthcare for more than 9,000 individuals.
The agreement will cover enrollees in Oxford Health-branded plans and UnitedHealthcare's employer-sponsored plans. The collaboration focuses on prevention, improved coordination and well-managed care, according to the news release.
Montefiore is also a participant in CMS' Pioneer ACO program.
