Humana, Boulder Community Health to partner for accountable care and population health

Boulder (Colo.) Community Health announced Thursday an accountable care agreement with Humana to provide more coordinated care for Boulder County residents with Connect for Health Colorado plans and Humana HMO plans.

The agreement focuses on preventive care, chronic conditions and disease management, according to the report. It will allow Humana beneficiaries to receive in-network access to inpatient, outpatient and emergency care services at Boulder-based Foothills Hospital, Boulder Community Hospital, the urgent care center and physician offices, in addition to specialty services at Mapleton Center, a Boulder-based outpatient rehab facility, and Lafayette, Colo.-based Community Medical Center. Humana members will have in-network access to more than 128 Boulder Community Health physicians.
