Anthem Forms ACOs With UCSF, Brown & Toland and Sutter Health Foundations

Anthem Blue Cross has announced that the University of California, San Francisco Medical Group, Brown & Toland Physicians, and various medical foundations linked to Sacramento, Calif.-based Sutter Health have agreed to join regional Anthem accountable care organizations that focus on patients with two or more chronic conditions, according to an article in the San Francisco Business Times.  

The Sutter medical foundations include the Palo Alto (Calif.) Medical Foundation, the Sutter Medical Foundation, the Sutter Gould Medical Foundation, the Sutter Pacific Medical Foundation and the Sutter East Bay Medical Foundation.

Each entity has its own separate ACO arrangement with Anthem. The ACOs give enrollees "access to a personalized health team including a doctor, care coordinator and other practitioners need," Anthem told the San Francisco Business Times.

The ACOs are available to fully ensured Anthem PPO patients with multiple chronic conditions.
