The following list includes accountable care organizations and collaborative care agreements that were announced, launched or expanded in August, beginning with the most recent.
1. Cedars-Sinai, CareMore Health to coordinate care for seniors in LA
Los Angeles-based Cedars-Sinai and the Medicare Advantage CareMore Health Plan announced an agreement to improve care coordination in order to keep patients healthy and out of the hospital.
2. Humana, King's Daughters Health partner for accountable care
Humana and King's Daughters' Health in Madison, Ind., announced a value-based agreement to improve health and better coordinate care for Medicare Advantage beneficiaries.
3. Aetna, MemorialCare to launch accountable care collaboration
Aetna and Fountain Valley, Calif.-based MemorialCare Health System announced a collaboration to offer accountable care to patients in Orange County and parts of Los Angeles County through a new commercial product, Aetna Whole HealthSM – MemorialCare.
4. Walgreens, Providence Health to coordinate care through retail clinics
Deerfield, Ill.-based Walgreens and Seattle-based Providence Health & Services announced a clinical collaboration to offer coordinated patient care through 25 new retail clinics in Oregon and Washington.
5. North Shore LIJ-Empire BlueCross BlueShield to launch ACO
Great Neck, N.Y.-based North Shore-LIJ Health System and Empire BlueCross BlueShield announced an agreement to launch an ACO for 33,000 commercial and Medicare Advantage members in New York.
6. Aledade to launch ACOs in 7 states
Bethesda, Md.-based Aledade announced it applied to CMS to launch ACOs next year in seven states, including Florida, Kansas, Louisiana, Mississippi, Tennessee, Virginia and West Virginia.
7. Reliance ACO joins Rite Aid Health Alliance
Camp Hill, Pa.-based RiteAid announced Farmington Hills, Mich.-based Reliance ACO will become the eighth partner part of the Rite Aid Health Alliance initiative for coordinated care.
